Acknowledgment and Assumption of Risk:

I, the undersigned, as the parent or legal guardian of the above-named child, acknowledge that participation in the event organized by Sovereign Community Outreach (SCO) and/or Sovereign Worldwide involves physical activities that may carry inherent risks of injury. I understand and accept that there are potential risks involved in my child's participation in these activities, including but not limited to physical injury, psychological stress, and other health-related issues.

Medical Treatment Consent:

In the event of an emergency, I give my consent for SCO and/or Sovereign Worldwide staff to obtain medical treatment for my child if necessary. I understand that every effort will be made to contact me in such an event. I agree to be responsible for any medical expenses incurred.

Release and Waiver of Liability:

In consideration of my child’s participation in this event, I hereby release, waive, and discharge SCO, Sovereign Worldwide, their officers, employees, volunteers, and agents from any and all liability, claims, demands, actions, and causes of action arising out of or related to any loss, damage, or injury that may be sustained by my child, or to any property belonging to my child, while participating in this event.

Indemnification:

I agree to indemnify and hold harmless SCO and/or Sovereign Worldwide from any loss, liability, damage, or costs, including court costs and attorneys' fees, that they may incur due to my child's participation in this event.

Photography and Media Release:

I consent to the use of any photographs, videos, or other media taken of my child during the event for promotional purposes by SCO and/or Sovereign Worldwide. I understand that these materials may be used in various publications, public affairs releases, recruitment materials, or other related endeavors.

Parental Consent:

I, the undersigned parent or legal guardian, certify that I have read this waiver and fully understand its terms. I understand that I am giving up substantial rights, including my right to sue. I acknowledge that I am signing this agreement freely and voluntarily, and intend by my signature to be a complete and unconditional release of all liability to the greatest extent allowed by law.
